Protection of Privacy Practices
CLIENT PRIVACY
L'Chaim collects personal information for the purpose of monitoring the health of our clients. We participate as a team with the families, doctors and healthcare authorities to provide our clients and their caregivers with the most thorough and compassionate care possible.
Each client is presented with a welcome letter which includes the following passage: PRIVACY
Respecting your privacy is important to us at L'CHAIM. The information we receive may be shared with professionals such as your doctor or case manager in order to provide you with the best care available.
We do take photos at L'CHAIM which sometimes appear in the JCC brochure or other publicity. Please advise the L'CHAIM administrator if you prefer that your photo not be used, or if you have any questions about L'CHAIM'S protection of privacy policy.
Clients (or their caregivers, when appropriate) are asked to sign the letter and a copy is kept on file by L'Chaim.
DONOR PRIVACY
L'Chaim collects donor addresses and credit information for the purpose of supplying tax receipts and the informational mailings of the L'Chaim Adult Daycare Society. The L'Chaim Society does not rent, sell or trade its donor mailing list.
